Along the same lines I have an application for my Palm Pilot called HealthFilePlus. It has both a desktop component as well as the PDA component. It can track the above mentioned items as well as glucose. weight, surgeries and other procedures, as well as the names of your doctors and clinics. I,m not sure if I can say how to get this app. so email if your interested.
