Pros: Lasts longer than Detemir Very steady glucose lowering effect from 3 to over 24 hours after injection. Glucose lowering effect tends to be very consistent from day to day. May be taken any time of day (but at the same time of day). Useful as a supplement to existing basal insulin (from the pancreas) for Type 2 patients.
Cons: Type 1 basal (background) insulin need is not steady during the day. Basal need is higher in the early morning because of the Dawn Phenomenon, and lower in the afternoon because of increased activity while awake compared to sleep, so a carefully adapted NPH regimen, with a larger dose at bedtime than in the morning, covers this varying basal need better than Glargine.
Overall: Be sure to test any Basal Insulin regimen while fasting. Rather than just looking at BG in the morning, compare BG at bedtime with the morning, and the morning with afternoon (and afternoon with bedtime), to see if these readings remain the same WHILE NOT EATING AT ALL.By
