is the effectiveness of my insulin depend on where I inject? Arms, legs, butt, belly. What seems to work the best?

All those places are good, as long as you inject into fatty tissue. If you have skinny arms then you might find the absorption of the insulin to be slower. Be sure to rotate sites like Harry suggested so you can avoid scar tissue. Many years ago I did not know about scar tissue and doctors did not tell me rotate sites. My upper abdomen now has permanent scar tissue and I can never use it again. I now rotate from my lower abdomen to my upper legs.

I switch mine every week one week in the belly next week in the leg my endo said to do that so the skin don't get hard.

My favorite place is my belly area. What I have learned medically and personally is the closer to the pancreas the better but the other sites are just as good. Every person is going to be different in how well a cite works. It never hurts to try and see what does and does not work for you.

No, but the time it takes to work might vary. There is no set scale but you may find that injecting in your belly works faster than your leg or similar.
