Medical condition/medication id card.
Where can one get a wallet size identification card that will list both medical conditions and medications. The card should be a compact but durable card that will withstand wear and tear.

As carol said laminate it. Any copy print store can do it for a buck
I posted this already a few days ago, you can look for it. Free medical ID card.
You can go to the site MedIDs.com and on the left side column scroll down and you'll see the ID card. The free one you fill it out on line and then print it . You can laminate it to be duable, and go back and change medications, drs info etc. You DONOT have to join or pay for it. You can see a sample card before filling yours out if you want.
It is great, and you have it instantly!
American Association of Diabetes has one. Go to their website and ask for info to be sent. In it is a medical id card wallet sized. I have one in my wallet. Hope this helps.
medicalnewscenter.com/emergencycard here is another one

www.usb-medic-alert.com I hope that this will be of some help to you.
Don't know, but someone else probably will. If not, type and print one of your own and then have it laminated for protection.
